FIVA ID Card Process
In accordance with the Technical Code of FIVA, an application must be submitted to the ANF (National FIVA Authority) of the country where the vehicle has a current registration. If the vehicle is not road registered, the application must be submitted to the ANF of the country where the owner is a legal resident. In this case, for the United States and Canada, the ANF is the Historic Vehicle Association (HVA).

- For Canadian residents OR those who would prefer to fill out a hard copy of the application, please click the link below for a printable version of the application. This may then be completed and submitted along with the corresponding documentation.

Instructions:
- The form must be completed in full: please do not leave spaces blank – if necessary write ‘none’ or ‘not applicable’ in spaces you may have otherwise left blank. Please quote dimensions in metric units for everything except wheels and tires, and please do not answer ‘standard’ or ‘unknown’.
- The online or paper application must be physically signed by the owner of the vehicle to be considered valid.
- If available, please provide a copy of your vehicle registration document with your FIVA application.
- The fee for the issuance of an Identity Card is $275. The total includes a Registered Membership into the HVA. Payment can be made via check and mailed to the HVA at the address listed below. Please make check payable to the Historic Vehicle Association.
